Use this to separate an AI-generated answer from the evidence needed before acting on it.

- [ ] State the exact claim or decision the answer supports.
- [ ] List the source for each important claim.
- [ ] Open the most important original or official source.
- [ ] Check the source publication or update date.
- [ ] Confirm that the source supports the exact claim, not a narrower statement.
- [ ] Mark any inference made by the AI or reader.
- [ ] Check one independent source when the decision has material consequences.
- [ ] Record conflicts instead of smoothing them into one conclusion.
- [ ] Check conditions that can change the answer: location, plan, user type, policy, and date.
- [ ] Escalate health, legal, financial, safety, security, employment, customer-impact, or compliance decisions to an accountable source or person.
- [ ] Save the source links and the remaining uncertainty with the decision.
